Job Summary

The Nurse Practitioner (NP)/Physician Assistant (PA) will play a crucial role in coordinating and managing care for patients with vascular and diabetes-related foot complications in the DRH Threatened Limb Clinic and across the Duke system. This position requires collaboration with vascular surgery, podiatry, wound care, infectious disease, and endocrinology teams to optimize care of the threatened limb patient, and to ensure seamless care delivery between the hospital and clinic settings. This position will be responsible for managing the care of threatened limb patients in the vascular surgery clinic, coordinating patient schedules across the multi-disciplinary threatened limb clinic (podiatry, vascular and wound care), and assisting patients in navigating care to optimize their outcomes. This position is also responsible for coordinating care between the inpatient and outpatient settings at DRH, and across the Duke Health System.

 

Job Duties and Responsibilities

  • Evaluate and manage vascular surgery and podiatry patients with peripheral artery disease and diabetes in clinic for initial assessment, planning procedures, post-operative follow-up, and surveillance.
  • Schedule and coordinate patient appointments and follow-ups with vascular surgery, podiatry, and wound care within the threatened limb clinic. Ensure that follow up appointments with endocrinology, primary care and infectious disease are also scheduled/coordinated. Triage threatened limb referrals to the appropriate clinic within the appropriate time.
  • Act as a liaison between the hospital and clinic and specialties, facilitating effective communication among healthcare providers to ensure cohesive patient care plans.
  • Oversee the development and implementation of individualized care plans for patients, ensuring that all aspects of limb preservation are addressed.
  • Provide patients and their families with information and education regarding their treatment plans, procedures, and follow-up care.
  • Maintain accurate and up-to-date patient records, including treatment plans, progress notes, and communication logs.
  • Order and interpret appropriate diagnostic and laboratory tests.
  • Perform procedures specific to the work environment after demonstrating competency and within the guidelines of the scope of practice and credentialing protocols. Document patient preparation and response to procedure(s).
  • Identify and coordinate access to necessary resources, including medical equipment, home health services, and community support.
  • Create and maintain a dataset for the threatened limb program to follow patient outcomes. Initiate and participate in quality improvement initiatives to enhance patient outcomes and clinic efficiency.
  • Work closely with the multidisciplinary team to develop and implement best practices for limb preservation and wound care.
  • Utilize prescriptive privileges for identified patient populations. Prescriptive authority for prescribing, ordering, and or administering drugs; including (but not limited to) schedule II, III, IV, and V drugs and necessary medical devices.
  • Utilize current research and evidence-based decision-making in all clinical practice. Incorporate clinical research findings in the development and implementation of standards of care.
  • Precept APP students and other new healthcare learners.
  • Participate in the development, review, and evaluation of medical management practice protocols within legal guidelines in compliance with DUHS policies.
  • Role model professionalism through conduct, appearance, communication, mutual respect, leadership, ethical decision-making, critical thinking, and problem-solving skills.
  • Demonstrate responsibility for professional practice through active participation in professional organizations and continuing education.

 

Job Eligibility Requirements

Education

  • PA – Graduate of an ARC-accredited Physician Assistant Program, master's degree preferred
  • NP – Graduate of an accredited MSN or DNP Nurse Practitioner Program for adults

 

Certification

  • PA -- PA-C
  • NP -- Adult providers, e.g., FNP, ACNP, AGACNP, AGNP, etc.

 

License

  • PA – Current unrestricted medical license issued by NCMB with authorization to practice as PA
  • NP – Current unrestricted RN License issued by or recognized by NCBON with authorization to practice as NP
  • Federal and State DEA prescriptive authorization

 

Experience

  • Strongly prefer candidate with 1+ year Vascular, podiatry, or wound care APP experience

               

Physical and mental requirements:

  • Able to clearly articulate scope of practice, and practices within those guidelines as a Nurse Practitioner / Physician Assistant in the state of North Carolina and within the Duke University Health System.
  • Able to independently seek out resources and work collaboratively to solve complex problems.
  • Able to communicate clearly with patients, families, visitors, healthcare teams, physicians, administrators, leadership, and others.
  • Able to use sensory and cognitive functions to process and prioritize information, and perform health assessments, treatment, and follow-up.
  • Able to use fine motor skills.
  • Able to record activities, document assessments & interventions; prepare reports and presentations.
  • Able to use a computer and learn new software programs.
  • Able to provide leadership in clinical areas of expertise and in meeting organizational goals.
  • Able to navigate the Hospital and DUHS to provide clinical expertise to specific patient populations
  • Able to withstand prolonged standing and walking with the ability to move or lift at least fifty pounds
  • Able to remain focused and organized
  • Able to work collaboratively with all levels of personnel

 

Job Hours

Schedule will be 8-5 Monday through Friday. Schedule may be modified by the clinical or leadership team to fit clinical needs of the service.
